Was sind Neurodermitis und atopisches Ekzem?

Neurodermitis wird synonym auch als atopisches Ekzem bezeichnet. Und hieraus abgeleitet sieht man schon, dass es sich um eine chronisch entzündliche Erkrankung der Haut handelt, bei der es in einer akuten Phase zu einer nässenden Hautentzündung kommt, die mit starkem Juckreiz einhergeht, und in der chronischen Phase des Ekzems ändert sich die Erscheinung hin zu einem trockenen Aussehen, wo die Haut trocken, schuppig und gerötet erscheint.

Wie zeigt sich eine erhöhte Neurodermitis-Neigung?

Es gibt sogenannte Atopie-Zeichen oder Neurodermitis-Zeichen. Diese sind einmal dadurch gekennzeichnet, dass sich eine papierartig trockene Haut zeigt, generalisierte Hauttrockenheit. Im Bereich des Gesichtes zeigt sich eine Ausdünnung der seitlichen Augenbrauen, eine Verdopplung der Unterlidfalte, dunkel umrandete Augen und im Bereich der Handinnenflächen zeigt sich eine verstärkte Handlinienzeichnung. Des Weiteren findet man eine Rauigkeit an den Streckseiten der Oberarme, hier erscheint die Haut sandpapierartig trocken.

Was sind die Ursachen für Neurodermitis?

Die Neurodermitis ist eine multifaktorielle Erkrankung. Hier spielen anlagebedingte genetische Faktoren sowie Umweltfaktoren eine Rolle. Auf der Seite der genetischen Faktoren haben wir über die letzten Jahre rausgefunden, dass einmal Veränderungen in Genen, die für die Hautbarriere wichtig sind, bei Neurodermitis-Patienten vorhanden sind und zum anderen, dass Gene verändert sind, die Entzündungsreaktionen beeinflussen, also Gene, die unser Immunsystem regulieren.

Auf Seiten der Umwelt ist es einmal so, dass Patienten mit Neurodermitis Allergien entwickeln, zum Beispiel gegen Hausstaubmilben, Birkenpollen, Katzenhaare oder Milcheiweiß und dass sie zum anderen auch empfänglicher sind, unter Stressphasen deshalb auch Neurodermitis, Schübe des Ekzems zu entwickeln und dass sie eine erhöhte Besiedlung der Haut mit mikrobiellen Faktoren haben, das heißt mit Bakterien, mit Pilzen, aber auch mit Viren, die dann dazu führen, dass ein Neurodermitis-Schub ausgelöst wird.

Infos zur Person

Mein Name ist Bernhard Homer. Ich bin Professor für Dermatologie und Direktor der Hautklinik des Universitätsklinikums Düsseldorf sowie der Sprecher des Universitäts-Allergiezentrums. Seit über 20 Jahren beschäftige ich mich wissenschaftlich mit den Grundlagen der Diagnostik und mit der Behandlung allergischer Erkrankungen, insbesondere des atopischen Ekzems oder der Neurodermitis.
